Ways to obtain a WIN/LOSS Statement

IN PERSON: Complete the WIN/LOSS request form available at either Queen’s Club location and present the clerk with your picture I.D. to ensure correct account information is retrieved. The WIN/LOSS statement will be available at the Queen’s Club Guest Pavilion within FOUR (4) business days. Your state issued picture I.D. must be presented at the time of pick up to ensure security of your personal information

BY MAIL: Send a self addressed stamped envelope, a letter stating that you are requesting your 2007 WIN/LOSS statement and a copy of your state issued picture state I.D. to:

RE: WIN/LOSS REQUEST

BY FAX: Fax a copy of your state issued picture I.D. and sign your name on the request for your 2007 WIN/LOSS statement to (618) 874-3036 and write “ATTN: WIN/LOSS REQUEST” on the front. Your statement will be available at the Queen’s Club in the Guest Pavilion within FOUR (4) business days or it can be faxed back- provided your return fax number is included with your request.
